About the Role – Choices at Home Team Leader

Are you passionate about supporting older people to live independently and stay connected to their community? As the Choices at Home Team Leader, you’ll lead a dynamic team delivering essential support services through the Commonwealth Home Support Programme (CHSP).

Your key responsibilities will include:

  • Planning and coordinating service delivery in line with contracts and budgets
  • Implementing processes and procedures to comply with reforms and aged care quality standards
  • Promoting Choices at Home to increase awareness and engagement among seniors
  • Building partnerships with local NGOs, health professionals, and community groups
  • Coordinate team processes for client service agreements, onboarding, assessments, referrals and addressing service gaps
  • Recruit and coordinate service volunteers

What You’ll Bring

To be successful in this role, you’ll need:

  • Diploma of Community Services (or equivalent)
  • Proven experience in team leadership
  • Strong background in aged care service delivery
  • Excellent organisational and administrative skills
  • Current Driver’s Licence and First Aid Certificate
  • Ability to obtain a satisfactory criminal records check
