Weekly Hours: 38 (flexible working arrangements available)
Hourly Rate: $1,307.86 - $1,425.57(plus superannuation)

About the role
This role is about making a difference in the lives of seniors in the community by coordinating and promoting a range of support services to help seniors remain active, healthy and engaged, whilst living in their own homes and community. It is about engaging with seniors for the design and implementation of services available through the Commonwealth Home Support Programme (CHSP).

Job/Responsibilities
- Coordinate Commonwealth Home Support Programme (CHSP) Services and other support services where required
- Undertake client assessment, make appropriate referrals for support services, and identify service gaps by researching the needs of the target group
- Plan service activities with the Choices at Home team
- Assist to recruit and support volunteers for the service and provide them with supervision and direction
- Meet the administrative and reporting requirements of the service and deliver the service within budget
So what do you need?
- Certificate IV in Community Services or relevant field of study
- Experience in a similar position including the delivery of services to a target group
- Current Drivers Licence
- First Aid Certificate
- Ability to maintain a Satisfactory Criminal Records Check
