About the role

This role focuses on positively impacting the lives of seniors within the community by delivering services associated with the Commonwealth Home Support Programme (CHSP). Responsibilities include providing individual and group social support services, such as escorted shopping and organised group outings, as well as transportation to appointments and activities.

The role entails engaging with seniors to create personalised home care plans following assessment for services. Additionally, it involves coordinating services that enable seniors to maintain an active, healthy, and engaged lifestyle while residing in their own homes and community.

Job/Responsibilities

  • Coordinate Commonwealth Home Support Programme (CHSP) Services and other support services where required
  • Undertake client assessment, make appropriate referrals for support services, and identify service gaps by researching the needs of the target group
  • Plan service activities with the Choices at Home team
  • Assist to recruit and support volunteers for the service and provide them with supervision and direction
  • Meet the administrative and reporting requirements of the service and deliver the service within budget

So what do you need?

  • Certificate IV in Community Services or relevant field of study
  • Experience in a similar position including the delivery of services to a target group
  • Current Drivers Licence
  • First Aid Certificate
  • Ability to maintain a Satisfactory Criminal Records Check
